摘要
尽管浅表性（皮肤/皮下）孤立性纤维性肿瘤(solitary fibrous tumor，SFT)已有报道，但因其与其他肿瘤存在形态学重叠，准确诊断仍十分困难。我们报道了迄今为止最大宗系列的浅表性SFT病例。纳入标准为SFT出现在真皮或皮下组织中。共确定26例病例。患者年龄范围为16~80岁（平均，47岁），女性明显居多（19例女性；7例男性）。肿瘤位于头（11例）、大腿（7例），背部(3例）、肩部（2例）、上臂（1例）、踝部（1例）和大拇趾（1例）。肿瘤平均大小为2.9cm（范围，1.0～7.0cm）。大多数病例（19例）具有典型的组织学特征（富于细胞性SFT）伴不规则梭形细胞束、鹿角样血管以及不等的胶原纤维。3例见明显坏死（全部<25%）。核分裂计数0～10个/10HPF(平均，2个/10HPF)。18例中有17例STAT6阳性，22例中有21例CD34阳性。参照Demicco及其同事提出的标准，所有病例（23/23）均为低危险度，包括2例显示恶性形态学病例。3例因缺乏肿瘤大小数据不能进行危险度评估。7例具有随访资料（平均随访时间，100个月：范围，2～241个月）患者均未见复发/转移。皮肤SFT更常见于女性，好发于头部。尽管需要更大宗病例研究的进一步证实，但根据现有标准，浅表性SFT通常为低危险度，表现惰性生物学行为。认识到SFT可以表现为浅表肿块，这会避免将其错误地归类为皮肤和皮下组织中经常出现的其他CD34阳性肿瘤。
