■57 y female patient, 60 py nicotin
■At the begin of 11/07 distal accentuated numbness, nearly daily synkope, sensible ataxia
■E´phys: sensorimotor polyneuropathy 感觉运动神经病
■Positive for anti-Hu antibodies
■SCLC right mediastinal, lymph-node metastasis SCLC右侧纵隔，淋巴结转移
副瘤综合征（红色为典型综合征）
■CNS 中枢神经系统
Encephalomyelitis 脑脊髓炎
Limbic Encephalitis 边缘性脑炎
Brain stem encephalitis 脑干脑炎

Diagnostic 诊断标准
■肯定的PNS
▷典型综合征伴出现神经系统症状5年内发现肿瘤
▷非典型综合征在肿瘤治疗后缓解或改善，而同时没有使用免疫治疗
▷非典型综合征伴肿瘤神经抗体，肿瘤在出现神经系统症状5年内发现
▷虽未发现肿瘤，但有典型或非典型综合征伴肿瘤神经抗体
■可能的PNS
▷典型综合征，未检出肿瘤神经抗体，虽未发现肿瘤，但有肿瘤的高危因素
▷虽未发现肿瘤，但有典型或非典型综合征伴肿瘤神经抗体
▷非典型综合征，未检出肿瘤神经抗体，在出现神经系统症状5年内发现肿瘤

■X-ray/CT-Thorax X-ray/胸透

■Gynecological/urological examination 妇科/泌尿科检查

Therapy 治疗
■Highest priority: rapid tumor treatment
首要任务：肿瘤的快速治疗
■no approved immunotherapeutical option (in particular concerning the CNS)
没有获批准的免疫治疗方案(特别是中枢神经系统)
■ probatory (off – label):检验（无标签）：
▷High-dose steroid-impulse-therapy, if ineffective 如果无效，高剂量类固醇脉冲治疗
▷IVIG, if ineffective IVIG，如果无效， IVIG
▷Cyclophosphamid (after consutling the oncologist)环磷酰胺（咨询肿瘤科医生后）
