糖皮质激素糖皮质激素glucocorticoid简称“皮质激素”。肾上腺束状带细胞所合成和分泌的激素。有氢化可的松和可的松等。主要影响糖和蛋白质的代谢,而对水盐代谢影响较小。且能对抗炎症反应,临床应用较广。其分泌和生成受腺垂体分泌的促肾上腺皮质激素(ACTH)所调节和控制。当机体中糖皮质激素过多时,反过来可以抑制腺体产生ACTH,两者互相影响,维持着动态平衡。一般可的松和氢化可的松可由机体内合成,亦可由人工合成。近年来通过化学方法在可的松和氢化可的松结构上加以改变,合成30多种可的松类衍生物,用于临床者约十余种,如强的松、强的松龙、甲基强的松龙、氟强的松龙、氟羟强的松龙、对氟米松、倍他米松、地塞米松等。临床主要利用它超生理剂量的抗炎、抗过敏、免疫抑制、抗毒和抗休克等药理作用来治疗一系列疾病。主要适合于严重中毒性感染、结缔组织病、过敏性疾病、肾病综合征、血液病、移植排斥反应的预防以及局部外用等。长期大剂量使用可引起类肾上腺皮质功能亢进综合征、骨质稀疏和肌萎缩、类固醇性糖尿病,诱发和加重溃疡和出血、穿孔,并发和加重感染及精神病。停药不合理可致医源性肾上腺皮质功能不全、激素停用后综合征和反跳现象。因此严重精神病、癫痫、活动性消化性溃疡、糖尿病、严重高血压、骨折、创伤修复期、恶病质,抗菌药物不能控制的感染(如水痘、麻疹、霉菌感染等),均应禁用皮质激素。
